I make and use 45 cal PH's, I don't think the bullet has ever come out the other side, then again I always shoot for the head. I would think the bullet does some damage ( just like a spear thru the head would do some damage) but most likely it gives a nice path for the gasses to expand into. As for blanks, don't they have less powder in them? The other thing is, the more you restrict an explosion the more powerful it explodes ( kinda like raising the compression in a hot rod) and pushing a piece of lead out of the shell, VS a paper wad, would add to the compression effect.
